What should I bring to my appointment?
It's very important that you bring your current medical insurance card or information, and a list of current medications or all your current containers of medicine to every appointment. Also bring a list of any vitamin or herbal supplements with their doses you are taking.
If you need to be cleared to play a sport, bring the sports physical form from your school.
If you have a work related injury and need to be examined, bring your worker's compensation (find at http://www.lni.wa.gov/FormPub/ ) information and date of injury (DOI) report.
If you are being licensed to drive large vehicles such as semi-trucks and need clearance, bring the Department of Transportation (DOT) form from the Department of Motor Vehicles (DMV). Find forms at http://www.dol.wa.gov/forms.html )
When I'm making an appointment, why do I need to tell the receptionist what I want to see the doctor for?
Appointments are made based on the length of time needed to discuss your medical concerns. Up to two concerns may be effectively dealt with during a 15 minute appointment. If you have more than two concerns, or if you have a complicated medical history, a longer appointment is needed. When patients come in with more concerns about either themselves or family members than time is allotted for, the physician may get behind and cause longer waiting times for the remainder of that day's scheduled patients.
When is a good time to come in for a blood draw?
Normally, Tuesday and Thursday mornings, and Wednesday afternoons are the best times to come in because only one doctor is seeing patients during these times. This frees up the medical assistants to be available for walk-in patients who need blood draws. Patients who have scheduled appointments will always be staff members' first priorities, however, they will make every effort to attend to patients needing blood draws as quickly as possible. Your patience is requested.
Why do I need to see the doctor at least once a year?
Your medical condition can change without you experiencing any particular ill health. In order to provide the best quality preventative care, the doctors would like to monitor your medication list and your blood work at least once a year. The clinic's policy is that if a patient has not been seen by the physician in over one year, that patient's prescriptions will not be automatically renewed, unless previously approved by the physician
